This is a Yohji Yamamoto y'sformen ink black over-sized short-sleeved shirt.

The material tag is faded and unreadable, but based on the feel, I believe it's crisp cotton.
Condition
As shown in the 6th photo, it's a vintage item, so there is a few general wear and tear, dust, and fading, but it's still wearable.
